At the time the public didn't trust the mysterious ingredients of a hot dog. So Nathan's hired a group of men and dressed them in white clothing, lab coats, etc and had them stand in front of the place eating hot dogs. The public assumed the white-clad customers were doctors from the neighboring hospital.
The conclusion was: if the food was good enough for the doctors then it was good enough for all.
